Learning Outcomes

Students will learn to describe the idea behind case management, its principle and processes and they will become familiar with methods in case management and networking, used in counselling indidividual clients and groups. Students will learn to describe in theory what case management involves, involving the assessment of clients' life situation and service needs; planning, implementation and follow-up of services,;multiprofessionalism and multiple providers of services; the role of the case manager in the coordination of client services. Students will become aware of various levels of case management. They will be able to theoretically analyse factors related to the needs assessment and provision, coordination and follow-up of services for clients, who require many forms of support. Students will become familiar with the settings and structures that provide services for elderly clients (in the public, private and third sectors) and with relevant social, health and rehabilitation laws, so that they can apply their knowledge to case studies. Students will understand that psychosocial and empowering elements are an essential part of case management.

Contents

Lectures, study of relevant laws, independent study. Students produce a manual based on a case study, in which they describe a problem and its background factors and the case management process, including important aspects at various points of the service pathway.

Topics may involve for example Old people's depression, loneliness, rehabilitation after above-knee amputation following diabetes, abuse, excessive use of alcohol, stroke rehabilitation, cancer etc.
