Learning Outcomes

Students are able to explain the aims and principles of assessing clients' needs for services. They have knowledge of the principles and methods used in drawing up care and service plans. Students are aware of the role of multiprofessional work in the assessment of service needs.

Assessment Criteria

Grade 5:
Students know how to analyse and assess clients' need for services. In co-operation with the clients, families and collaboration partners, students are able to draw up a care and service plan to meet clients' individual needs and to promote their social engagement with help of public, private and third sector services.
Grade 3:
Students know how to compare the client's need for services. They apply the principles of case management in drawing up care and service plans.
Grade 1:
Students recognise clients' needs for services. They are able to name principles of case management, used in drawing up care and service plans.
