Learning Outcomes

Students will learn long-term case management and principles of holistic service planning. They will be competent in planning service packages to meet client needs with help of local service provision. Students will also be competent in identifying the worker's role and practices throughout the case management process. They will know how to promote the implementation of the planned services by means of collaborative networks. Students will learn to integrate the principles of networking and family group conference into case management practices.

Lectures 34 hours, independent study and exam 70 hours.

Contents

- Concept and principles of case management
- Client-centred assessment of needs
- Integration of services and the crossing of sectoral borders, pathways of services
- Identification of clients' social networks
- Courses of action in networking and the use of networks
- Techniques for networking, e.g. family group conference, network meetings, network map

Assessment Criteria

Grade 1-2:
Students recognise the stages and principles of a goal-oriented case management process. They know the most important methods of case management and are able to apply them to case studies. Students are able to contribute to case management planning as members of a work group.
Grade 3-4:
Students have command over case management as a method in social services. They know the most important methods and are able to apply them and justify their use in social field work. Students are able to assume responsibility for case management planning and implementation.
Grade 5:
Students are able to extensively and profoundly evaluate the background ideas and significance of case management as a method. Students know how to apply and develop case management methods. They are able to assume responsibility and encourage other members of a work group in case management planning and implementation.

Assessment Methods

Assessment on a scale of 1 to 5 will comprise an exam and independent assignments.
